Registering at Kinmylies Medical Practice

Patients are asked to complete a registration form and will be offered an appointment with a nurse for a 'new patient check'. You will be registered with the practice, but not with a specific doctor.

Registration forms can be downloaded for printing by clicking the links below. Both forms should be completed and taken into the practice.
